Prisons
National TB Management Guidelines, 2018 Edition. Chapter 3, Systematic TB screening in high risk groups.
Clinical description
- The target population are prisoners (depending on circumstances may include warders
and other relevant prison staff)
- Both symptomatic screening and x-ray will be used for screening where available.
- All prisoners will be screened at entry (symptomatic screening), then every six months
and during exit.
- Prison clinicians, patient attendants, peer educators, lab personnel and x-ray
technicians will take part in implementing this intervention.
- Xpert will be used as the first test for presumptive TB cases in prison settings.
3.5.5 Intensif ied case f inding (ICF among PLHIV)
- ICF is implemented in HIV clinics (service providing care for PLHIV)
- The target population is PLHIV and in HIV care.
- The 4 symptom screening criteria will be used
- Screening criteria / algorithm: 4 item screening criteria for PLHIV (see Table 4-1)
o Cough (any duration)
o Fever (any duration)
o Night sweats (any duration)
o Weight loss
- Screening should be at every follow-up visit
- A multidisciplinary team in the health facility (TB, ART, laboratory, facility in charge,
OPD clinicians) should ake part in implementing this intervention.
- The multidisciplinary team is required to meet regularly and document the meetings.
- Proper documentation of presumptive TB cases: ensure utilization of chronic cough
register in a facility to monitor proper implementation.
- As per WHO recommendation Xpert, where available, will be used as the initial test for
presumptive TB cases among PLHIV. (See diagnostic algorithm, Figure 3.31).
