Post Discharge Malaria Continuum of Care (PDMCC)

Guidelines for the Treatment of Malaria in Malawi, 6th Edition, 2025. Chapter 1, Severe Malaria.

Treatment

Post Discharge Malaria Continuum of Care (PDMCC) is the continuous administration of a full antimalarial treatment course at regular intervals to under five children discharged from hospital with severe malaria or severe anaemia.

Recent studies in Malawi and elsewhere demonstrated that within six months after being discharged from hospital for severe anaemia/malaria, these children have five times risk of either readmission or death from the same conditions. This treatment is given within a period of three consecutive months, starting two weeks later for those discharged on LA, and four weeks later for those discharged on DP.

PDMCC is highly effective as it reduces the risk of severe malaria, severe anaemia related to malaria and deaths by 87%, 89% and 70% respectively.

MoH recommends the use of DP for PDMCC as continuation of care. PDMCC is delivered in two models: either through a health facility or community model. The health facility model is delivered through nearby hospital or health centre. The community model is delivered through village clinics. In the village clinics, PDMCC is provided by iCCM providers while at health facilities, it is provided by clinicians and nurses.
